AI Project Postmortems: Lessons from What Went Wrong
Nobody wants to talk about failed AI projects. Vendors don’t mention them. Consultants quietly remove them from case studies. Internal teams bury them. But failure patterns are often more instructive than success stories.
Based on postmortems I’ve conducted or reviewed, here are common patterns in AI project failures – and how to avoid them.
Pattern 1: The Solution Looking for a Problem
What happened: A technology team got excited about AI capabilities and built an impressive demonstration. They then searched for a business problem it could solve. When they found stakeholders willing to sponsor a pilot, the solution never quite fit the actual need.
Example: A sophisticated NLP system for customer feedback analysis that produced insights nobody used because it didn’t connect to any decision-making process.
Root cause: Technology-first thinking. Building capability and then seeking application, rather than starting from business problems.
How to avoid: Always start with the business problem. What decision will this improve? Who will use the output? What will they do differently? If you can’t answer these questions specifically, stop.
Pattern 2: Data Denial
What happened: A project was approved based on assumptions about data availability and quality. When the team actually accessed the data, they found it was incomplete, inconsistent, poorly documented, or structured differently than expected. The project spent its budget on data preparation and ran out of resources before delivering value.
Example: A demand forecasting project that assumed clean historical sales data. Reality: sales data was scattered across three systems with different product codes, missing two years of history from an acquisition, and contaminated with promotional data that wasn’t flagged.
Root cause: Insufficient due diligence on data before project approval. Optimistic assumptions that weren’t validated.
How to avoid: Conduct data assessment before project approval, not after. Actually look at the data. Talk to people who work with it. Assume it’s worse than described. Budget data preparation as a significant project phase.
Pattern 3: Scope Creep to Death
What happened: A project started with a focused scope. Early successes led to expanded requirements. Stakeholders added features. The team accommodated requests to maintain support. The project became so broad it could never be completed. Eventually, budget exhausted, nothing was delivered.
Example: A document processing project that started with invoice automation, expanded to include contracts, then purchase orders, then expense reports, then “any document.” The system that could process invoices well never reached production because it was constantly being expanded to handle new document types.
Root cause: Failure to enforce scope discipline. Confusing stakeholder enthusiasm with project health. No explicit change control.
How to avoid: Define scope explicitly. Document what’s included and what’s not. Implement change control that requires trade-offs – new scope means extending timeline or dropping something else. Celebrate completing the defined scope rather than endlessly expanding.
Pattern 4: The Pilot Trap
What happened: A successful pilot was celebrated and then… nothing. Scaling to production required infrastructure, integration, change management, and operational processes that weren’t budgeted. The pilot demonstrated possibility but nobody funded reality. Years later, the pilot is still running in demo mode.
Example: A ML model predicting equipment failure that worked brilliantly in pilot with three machines. Scaling to 300 machines required data infrastructure, integration with maintenance systems, training for technicians, and 24/7 support. None of this was planned. The pilot remains a pilot.
Root cause: Treating pilots as endpoints rather than learning phases. Not budgeting for scaling before starting pilots.
How to avoid: Plan for success. Before starting a pilot, understand what production looks like – infrastructure, integration, operations, change management. Secure commitment (at least conditional) for production investment before starting. If you can’t, reconsider whether the pilot is worth doing.
Pattern 5: Vendor Overpromise
What happened: A vendor demonstrated impressive capabilities. The organisation bought. Implementation revealed that the demo was carefully orchestrated, edge cases were common, and getting the promised results required extensive customisation the vendor couldn’t deliver.
Example: A customer service AI that handled demo conversations brilliantly. In production, it couldn’t handle accents, got confused by simple variations, and escalated 80% of conversations. The vendor blamed the customer’s data. The customer blamed the vendor’s misrepresentation.
Root cause: Accepting vendor demos as representative. Insufficient due diligence. Contracts that didn’t protect against underperformance.
How to avoid: Test with your data, your users, your edge cases. Talk to references who’ve gone to production, not just pilot clients. Include performance guarantees in contracts with teeth – right to terminate, price adjustments, or support commitments if performance doesn’t meet agreed thresholds.
Pattern 6: Champion Departure
What happened: A project depended on a passionate executive sponsor. When that sponsor left the organisation, the project lost its protection. Budget was cut, resources were reassigned, and the project died despite technical progress.
Example: A transformational AI program with a CTO sponsor. When the CTO departed, the new CTO had different priorities. The program was defunded at 60% completion. Two years of work produced nothing.
Root cause: Single-point-of-failure for sponsorship. Insufficient organisational embedding.
How to avoid: Build broader sponsorship coalitions. Don’t depend on a single champion. Deliver value incrementally so the project can survive leadership changes. Make the project’s success important to multiple stakeholders.
Pattern 7: Integration Nightmare
What happened: An AI model worked well in isolation. Integration with existing systems proved far more difficult than expected. Legacy systems couldn’t accommodate real-time AI calls. Data formats required complex transformation. Security reviews blocked deployment. The model worked; the integration never did.
Example: A recommendation engine that required real-time access to inventory, pricing, and customer data. Each integration took months longer than planned. By the time integrations were complete, the underlying model was outdated and needed retraining. The cycle repeated.
Root cause: Underestimating integration complexity. Building AI in isolation from operational systems. Insufficient involvement of enterprise architecture and security teams.
How to avoid: Involve integration stakeholders from project start. Understand the integration landscape before building. Build integration into project timeline and budget realistically. Consider integration-first prototypes that validate connectivity before building sophisticated models.
Common Threads
Across these patterns, common themes:
Planning failures. Most failed projects were predictable. The issues that killed them were knowable early if anyone had looked.
Optimism bias. AI projects are particularly prone to “this time it’s different” thinking. It usually isn’t.
Technology focus over business focus. Projects that focused on technical achievement over business value delivered technical achievement and no value.
Change management absence. Even technically successful projects fail when they don’t achieve adoption.
Applying These Lessons
Before starting your next AI project:
- Can you articulate the specific business problem and how decisions will change?
- Have you actually examined the data, not just assumed it’s available?
- Is scope explicitly defined with change control?
- Do you have commitment for production, not just pilot?
- Have you verified vendor claims with your data and references?
- Is sponsorship broad enough to survive personnel changes?
- Have integration stakeholders assessed feasibility?
If any answer is “no,” address it before proceeding. These questions are easier to answer before starting than to solve mid-project.
Final Thought
Every experienced AI practitioner has war stories of failed projects. The patterns repeat because organisations don’t learn from others’ mistakes – or their own.
Postmortems are uncomfortable but valuable. Conduct them honestly. Share learnings broadly. The investment in understanding failure pays returns in future projects.
